Hello parents and guardians! Please fill out the following form to register your 5-7th grader for Kadima or 8-12th grader for USY this year! Please note that the form must be completed fully, and that membership is contingent on receiving a payment for youth groups. Please fill out one form per child. The cost of membership is $25/year/child, but if that proves to be a financial burden, please reach out and we can absolutely accommodate your needs.
